Three months before the start date of the J-1 Exchange Visitor's program--but no later than 60 days--you should submit the following documents to SISS:

1. Request for Form DS-2019 (typed) and Health Insurance Memo of Understanding
2. J-1 Category Selection Form (J Visa 5 Year Program Rule Online Course)
3. Documentation of funding (if not salaried by UC Davis)
4. Memo of invitation to J-1 Exchange Visitor from host faculty member
5. Copies of current passport and previous U.S. immigration documents
6. Attachment for J-1 Physician, if applicable
