Forms & Handouts

Overview

These forms and handouts are meant for studentsscholars, and departments at the University of California, Davis. If you have any questions regarding the use of request forms, please contact SISS (530-752-0864 or siss@ucdavis.edu).

Because of frequent changes to laws and procedures, you must use these forms as they are the most current information. SISS will disregard any requests submitted using previous versions of forms. 

  • Student Forms include: F-1 Employment, J-1 Employment, and general handouts (all student forms are in iGlobal)
  • Scholar Forms include: J-1 Scholars, J-2 Dependents, H-1B, E-3, O-1, TN, and general handouts
  • Department Forms include: E-3, H-1B, J-1, O-1, TN, and Permanent Residence

Government Immigration Forms

The U.S. government issues a variety of forms to students and scholars both directly and indirectly (e.g., through SISS). The following is a list of common forms.

  • Student Visa Certificates (Form I-20 / Form DS-2019) 
  • Form I-20 is a visa certificate needed to apply for an F-1 visa, and Form DS-2019 is used to apply for a J-1 visa and enter the U.S. Your I-20 / DS-2019 must also be carried and updated regularly throughout your time as a student.

  • Form I-94 Arrival Record 
  • Your Form I-94 arrival/departure record is verification of your legal status while in the U.S. It also shows the date your legal stay expires (“admit until” date). For F-1 and J-1 students and scholars, the expiration date is “D/S.” For other visa types, it may be a specific date. Your I-94 must reflect the correct visa type and must always be unexpired. Carry a copy of your I-94 at all times while in the U.S.

    If you changed your status since coming to the U.S., your new I-94 will be attached to a Form I-797A Approval Notice.

  • USCIS-Issued Employment Documents 
  • USCIS may generate different types of employment authorization documents depending on your situation. For example, Permanent Residents may be issued a Form I-797A showing successful approval for a Form I-485 adjustment of status, or a Form I-551 permanent resident card (“green card”).

    Non-immigrants in different classifications may be issued a Form I-766 Employment Authorization Document (EAD) card, also referred to as a work permit.
